1. As we age, the shape of our face changes.
Have you ever noticed that as we age, the beauty and good looks we used to have in the past change? These things can be easily explained using geometric shapes as follows:
Teenagers/Young adults > The face will be > Inverted triangle shape.
When we are young, the creation and repair of the various layers of the skin, including skin, collagen, connective tissue, and the strength of the muscle layer are still good. Therefore, if we put a triangle on our face, it will fit perfectly.
Older people > The face will be > Upright triangle shape.
But as we age, collagen is lost, the strength of the layers of the skin is reduced, and the face sags, resulting in an upright triangle shape.
Adolescence/Youth | Elderly |
– High cheekbones -With cheeks/orange cheeks -Clear face frame | – Under eye wrinkles, cheek wrinkles, and mouth corner wrinkles -Sagging cheeks - Double chin, sagging face, unclear facial contours |
2. The areas that will change the most are the midface and lowerface.
Midface area: covers from below the eyebrows to the corners of the mouth.
Lowerface area: covers from the corners of the mouth to the tip of the chin.

This area will change from
(2.1).Bone layer: Bone loss
As we age, the facial bones begin to sag, the ligaments that support the face become loose, and the fat and muscles are repositioned, making the face appear to droop. When the face droops, it descends straight down and inwards, resulting in under-eye wrinkles/bags, smile lines, and smile lines at the corners of the mouth. When we look from the side, we can clearly see that the face is sagging.
(2.2). Fat layer: Fat
Every 10 years there are changes in the deep and superficial fat layers, such as:
> Redistribution (change of position): The superficial fat layer moves from top to bottom.
> Accumulation (Accumulation): Accumulation occurs in the lower facial area.
> Atrophy: The deep fat layer under the eyes is lost. Deep fat is what supports the facial ligaments. When there is atrophy, what happens is that the ligaments sag, causing the face to sag.
(2.3).Skin layer: Skin Changes with aging
Starting from the age of 30, the skin gradually becomes thinner, more sensitive to the sun or develops abnormal pigmentation. Less collagen is produced, causing the facial skin to lose its firmness. The sebaceous glands atrophied and produced less water under the skin, causing the skin to lack moisture, resulting in small wrinkles.

3. Treatment guidelines to improve aging and Sagging appearance
(3.1).Filler :
By supporting the bone layer, the facial ligaments return to their original position, or what is called a face lift with filler.
And there is filling in the missing parts to make the proportion of the face look more beautiful, whether it is filling the filler under the eyes, cheeks, or enhancing the chin area to make the face look more dimensional, a beautiful S Curve.
(3.2).High Intensity Focused Ultrasound : HIFU
By sending ultrasound energy waves through the skin layer (Dermis), fat layer (Superficial fat), muscle layer (SMAS layer) to stimulate collagen production, making the skin firmer.
(3.3).Botulinum Toxin :
Helps correct wrinkles, lifts the face frame to make the skin firm and look younger, etc.
We will assess and analyze the root cause of the problem and fix the defects for the best results and a more beautiful facial proportion.
All of these are both science and art that expert doctors evaluate. Because in addition to accurate angles and the right proportions, doctors also consider the unique and distinctive beauty of each person.
